Care Coordinator

Permanent, Full Time / Professional / 2 years exp

 

The Care Coordinator is responsible for providing support to the clinical team in the assessment, case work, programmes and supporting the clients and their families to resolve personal, social, emotional, and financial issues.

 

He/She will provide service coordination management, administrative support, and operations improvement. The Care Coordinator will need to work collaboratively with colleagues from other departments to ensure a seamless client experience for our service users.

 

Job Responsibilities:

  • Support clinical team in advising clients on financial scheme applications
  • Ensure timely update of renewal of means testing and financial assistance
  • Support the clinical team in administrative processes in the delivery of services to clients and caregivers
  • Manage onboarding process for new clients, assist in initial assessment appointment scheduling / rescheduling, subsidies application, and address client inquiries for rehabilitation
  • Assist in initial assessment of client / caregiver needs with basic care management under the direction of a case manager / social worker
  • Ensure client and caregiver's ability to cope by helping to streamline and coordinate the range of community services available
  • Assist client / caregiver with appropriate information and referral assistance
  • Conduct joint home visits with a case manager / social worker or the multidisciplinary team where necessary
  • Participate in case discussion with the clinical team
  • Consolidate, prepare, track relevant monthly reports and statistics

 

Job Requirements:

  • Minimum Diploma in any discipline, prerably in healthcare or a related discipline
  • Team player with good interpersonal and communications skills
  • Patient, gentle disposition with empathy and strong listening skills
  • Able to converse in English and at least one other local language is preferred
